Age: If your character is not a human, give it in animal/fairy/alien/whatever years and human years—for any species you invented, calculate a constant (e.g. 0.6667 alien years per human year)
Height and weight: Give numbers if possible (see this chart for females and this chart for males) or use relative values (e.g. he's short for his age).
Body type: frame (thin, stocky, etc.), weight, anything else
Eyes: color, shape, eyebrows?
Hair: cut and color
Race: white, black, East Asian, etc. if human; describe any important racial traits if another race
Nationality: country of origin, current country, accent, anything notable
Birthday: Yes, not only do you have to remember your family members' birthdays, but you have to know your characters', too.

The higher a character's EQ is, the more other characters (and generally readers) are likely to like him/her. See the artist's description for more info.

MBTI: Google "Myers-Briggs free" if you don't understand this. This is a personality test I have all my major characters take. If your character can't take it properly, then that character probably isn't very developed. (That's normal for minor characters, but it's bad for major ones.)
Maturity: or psychological age
Relationship status: married, single and uninterested, dating _______... sexual orientation and relevant relationship history can be mentioned (e.g. she once had a mean boyfriend and is therefore cautious about relationships)
Social standing: popularity, reputation, etc.
Philosophy/Values: What is most important to your character? How does this affect his/her perception of the world?

Quirks and mannerisms: Often these make characters more memorable and realistic. (eg. she twists a lock of hair around her little finger when she thinks, he stammers when he's confused) If someone is in love with this character, he/she may lovingly notice these mannerisms.
Misc. Views: Record any views mentioned here. (e.g. she is a Republican and a Protestant, or he believes that everyone who likes chocolate ice cream is an idiot) Go ahead and be specific. This will record what your memory might lose. I recommend chapter citations, too, in case you want to see where you found something.
Minor Notes: If she says she doesn't like vanilla ice cream in chapter 2, she shouldn't be eating some in chapter 5.
